Pediatric massive transfusion remains a high-stakes, time-critical intervention, often guided by limited child-specific evidence. Recent work emphasizes standardized, weight-based protocols to improve adherence and streamline activation, while highlighting variability in current practice. Emerging strategies, including the use of whole blood and viscoelastic testing, aim to optimize hemostatic balance and improve outcomes. Together, these developments reflect a shift toward practical, data-driven transfusion strategies tailored to children.
