The purpose of this form is to collect proposal(s) for educational activities that will be seeking joint accreditation in 2024 and/or organizational resources such as AV/IT support and marketing. Submission of the Intent To Apply does not necessarily mean your activity will be approved by the committee and sent forward for accreditation next steps. To guarantee consideration, submissions for 2024 are taking place now through November 15, 2023. YOU WILL NEED TO FILL OUT A SEPARATE FORM FOR EACH PLANNED ACTIVITY. For questions regarding this process, please email education@childrensmn.org. If you are seeking AMA, ANCC, and/or ACPE credits for your activity, these activities will be managed and maintained in the continuing education learning portal.

Please indicate if you will be seeking assistance from marketing. You will need to fill out a separate request for marketing. Some examples marketing can assist with are: brochures, email communication, fliers, marketing pictures, and banners, etc.

Please indicate if you will be seeking AV/IT resources for your event, such as Zoom or TEAMS link, on-site assistance, etc. You will need to submit a separate request for these resources. Our teams do not provide IT/AV assistance.
