• Describe common challenges and barriers of transitioning to adult care
• Identify members of the care team and resources that are important to a successful transition from pediatric to adult care
• Introduce strategies or tools that care teams can use to help transition from pediatric to adult care, including overcoming active or passive resistance from patient/families
