Objectives: 

• Describe Children's Minnesota's approach and design for the 2025 Community Health Needs Assessment (CHNA).
• Recognize how community members and partners contributed to the 2025 CHNA, why their voices matter for child health, and what key themes and needs emerged from their input.
• Identify specific initiatives areas, goals and objectives included in the 2026-2028 CHNA Implementation Strategy and determine concrete ways clinicians can participate in addressing community-identified priorities.
