L042425 Clinical Ethics Case Review: Ethics of Consent in Urgent and Emergent Medical Interventions

A 14 year-old is transferred to the pre-op area from the ED with increased intracranial hypertension, likely due to meningitis, found on CT and clinical assessment. The child is moving some but is becoming increasingly obtunded. He still breathing on his own. When going through the procedure to place a drain to decrease the pressure in the child's brain the mother refuses to consent. She remains adamant that she does not want her child to have the surgery.

A 7 year-old with a diagnosis of sickle cell anemia is admitted for treatment of splenic sequestration. He is clinically stable at the moment.  The treatment is a blood transfusion. The father refuses based on the family's faith beliefs as Jehovah's Witnesses.

An infant in the NICU is scheduled for ventricular-peritoneal shunt and removal of the reservoir they currently have to treat their hydrocephalus. The parents are not at bedside and cannot be reached.

This month, we will review the ethical considerations around parental consent.  We will discuss the distinction between urgent and emergent and how clinicians respond to each scenario in an ethically appropriate manner. We will also discuss issues of consent when interventions are not urgent or emergent but the consenting parent is not present and delay of treatment does not cause immediate harm but is not ideal, not best for the child, and not within parents' goals of care for their child. We will identify the ethical considerations in each case and provide recommendations and guidance on how clinicians can approach these types of scenarios in practice.

Clinical Ethics Case Review will explore real cases to help clinicians understand how to identify, approach, and manage ethical issues. Every other month we will present a real case and review the ethical issues, provide ethical analysis, and discuss ways to approach these in practice. This educational opportunity will be beneficial for all clinical staff and especially for early career clinicians.

These activities will be held bi-monthly, virtual, 4th Thursday from 11:30 – 12:30 pm.
